Well I cannot say that I am entirely familiar with the topic, although I have heard snippets here and there. Additionally, I am personally a Christian and have gone to a Catholic school before.

I would say that the first thing that has to happen is a dialogue. This sounds cliche, but this is the truth. If the Catholic church is not willing to talk about issue ongoing within their own community, then its denial will perpetuate the issue. Also, people outside of the Catholic church need to talk about it, or again, the issue will go on.

Yes, in particular from the top to the bottom. For example, the Pope talking to bishops, who then talk to the fathers and priests within their regions. The church is a hierarchy in that they take following from above, and it being unlikely for a father to speak out on the issue without the pope first making it clear.

Because when everyone is talking about the topic, then everyone is aware. This creates accountability and making them responsible to report an infringement if they see it occur.
