Well, number 4 is asking why Toussaint L’Ouverture was captured. We know from the passage that Toussaint L’Ouverture was a former slave who gained control of an island and released all the slaves there. After that, it says that the French captured him because he was the leader of this revolt. Knowing this, we can assume that the French captured Toussaint L’Ouverture in an effort to stop the revolt from happening.

And for number 5, the last line of the orange text says "In 1804, Haiti declared its independence, thus making this the only successful slave revolt in history." Which makes the answer to number 5, Haiti.
