Firstly, it is possible that there has been a amalgamation-up or misunderstanding. The medicos may be referring to a Paul with a different last designation. Alternatively, they could be misconstrue or intentionally prevaricating about their relationship to Paul.

On the other hand, Paul may genuinely not be vigilant of any siblings. It is possible that he was adopted or dissevered from his siblings at a puerile age and ergo has no cognizance of them. Alternatively, he could be intentionally gainsaying their relationship for personal reasons.

It's also quite possible that Paul IS their brother because the doctors are his sisters and then that would mean that neither party is lying.
