whats the difference between equillibrium potential and standard potential..?

standard potential is determined by keeping the concentration of ion as 1M,pressure of gas at 1atm, and the measurement done at 25 degree celsius. when solution of an electrolyte contains more than 1 type of cations and anions at concentrations different from 1M, the discharge of an ion doesnot depend solely on standard potentials but also depends on the concentration of ion in the solution. this value is referred to as potential. relation between them is given by Nernst equation as

So equillibrium potential which is potential at equillibrium at any condition of temperature and pressure whereas standard potential is again an euillibrium potential but at standard condition....isn't it..? potential that we measure according to nernst equation..is potential wrt hydrogen ion..?
