I'm assuming the perfect square trinomial formula just means to complete the square (from the quick google search I did).

This means you factor out the 16, giving you 16[y^2-(1/2)y+(1/16)]=0, this gives you a perfect square trinomial, so you can see that it's just (16)[(y+(1/4))^2]
