1) A mouthpiece is a person or periodical that expresses the views of a group. 2) A spokesperson is one actually speaks for a person or a group. The difference here being that in #1 the views are only expressed or generalized and in #2 the person is speaking in the words of someone or some group.

Actually, they're both the same thing. "Mouthpiece" is just a slang, slightly derisive term for spokesperson, implying that the spokesperson lacks any capacity for independent thought.

A mouthpiece is the removable metal piece you blow into on a brass instrument. A spokesperson is actually a person. I'm not sure how the two can be confused, but I'm here to help not judge.
