Most of the time but not always. There are quite a few species of flukes and parasitic worms which are counter examples.

I wouldn't call that a disease. Its just how the cells look inside them can only be seen by a microscope.

the disease causing organism are not always the microscopic .they may seen with the naked eyes.like mosquitoes cause the malaria but some disease causing organisms are microscopic like virus which cause the polio.influenza and small pox etc.

the straightforward answer would be bacteria.
some tapeworms flukes etc may also invisible to the naked eye
virused can be seen only with electron microscopes, and indeed are invisible to the naked eye.
the smallest disease causing biological particle would be prions - misfolded proteins.
actually some inert compounds do cause disease and answer the same criterion - all sorts of inhaled dust particles that cause pneumoconioses - work related diseases
