Hmmm, this can be due to some error arises during DNA replication. Before cells divide, they need to double their DNA content so that the daughter cells can have same number of genetic material. Error in DNA replication arises can be due to transient base tautomery because all 4 DNA bases (A,T,C,G) are capable of either amino-imino or keto-enol tautomey. Therefore, each base can have at least 2 forms. Once 10^-4 per nucleotide, DNA polymerase adds wrong base because of this tautomerism. http://en.wikipedia.org/wiki/Tautomer
