y-intercepts are ALWAYS on the y-axis. The x-value is ALWAYS "0". The "form" is y = mx + b. That 'b' is the y-coordinate of the intercept.

You hae to get this concept in your head. That slope is a fraction, 1/4, 3/8, 4 = 4/1, -1/3, -5 = -5/1. Every "slope" can be expressed like this. How are we doing?

That fraction contains tons of information. We often hear it called "rise over run" or more technically "change in y over change in x" Thus, 1/2 means change 1 in the y direction for every change of 2 in the x direction. Positive is assumed to be up and right. Negative means down or left. -1/3 means change -1 (down) for every change of 3 in the x direction. 3 = 3/1 means change 3 (up) in the y direction for every change of 1 in the x direction. With this information, if you have one point to start (the intercept, for example), you can define the entire line.
