The point-slope form for the equation of a line is: \[y - y _{1} = m(x - x _{1})\]where\[x _{1} \]and\[y _{1}\]are from the cooridinates of your given point and "m" is the slope. Substitute and simplify.

btw, if you are not familiar with the form of the coordinates of a point, that is:\[(x _{1}, y _{1})\]You can match that up with the given point to determine which is the "x" and which is the "y".
