thomaster (thomaster):

A guy accidently pours boiling water over his legs. After about 15 minutes large burn blisters have developed on his leg. He went to the GP which wrapped his leg in bandage and prophylactic applied antibiotics. The next morning our guy wakes up with 38.5ºC fever and very moist wounds. 3 days later, the situation deteriorated. He suffers pain, 39.1ºC fever, his burn wounds are dirty, gray and the edges are swollen. What is the diagnose?

OpenStudy (anonymous):

feb that is fever already implies INFECTION the wound moist can be diagnosed "thrombocytopenia" that is no blood clottting the AGE of your guy his wounds are not healing after treatment meaning problem with clotting factors ALWAYS THE KEY FEVER IMPLIES INFECTION PLEASE CAN YOU COME ONLINE LETS DISCUSS THIS IN DEPTH I LOVE QS LIKE THIS

thomaster (thomaster):

He's 19 years old

OpenStudy (anonymous):

i just dont know what infection would casue wound non healing and pain also it could be auto-immune but we ll have to be sure

thomaster (thomaster):

It's for a microbiology assignment, all these assignments are about bacterial infections

thomaster (thomaster):

my guess would be something like Staphylococcus aureus or Pseudomonas aeruginosa since those can be part of the skin flora

OpenStudy (anonymous):

which one do you think it is streptococcus or staphylococcus

OpenStudy (anonymous):

strep is usually fever asymptomatic

OpenStudy (anonymous):

so it cant be strep

OpenStudy (anonymous):

now we are left with pseudomonas and SA

OpenStudy (anonymous):

which one is antibiotic resistant

thomaster (thomaster):

the fever is probably caused by endotoxins from gram negative cell walls

thomaster (thomaster):

And it causes thrombocytopenia

thomaster (thomaster):

so probably pseudomonas i think, they're notorious for causing burn infections i just read

OpenStudy (anonymous):

yeah gram negative bacteria like pseudomonas is the likelyhood is SAureus gram negative too pls refresh my memory i need to recall all my bacteria classes

thomaster (thomaster):

SA is positive

OpenStudy (anonymous):

i hope you re enjoying this as much as i do can we rule out SA

OpenStudy (anonymous):

which is most prevalent

thomaster (thomaster):

yes i'm allmost sure it's P. aeruginosa. A lot of articles about it infecting burns and cause thrombocytopenia

OpenStudy (anonymous):

fine there you have your answer

thomaster (thomaster):

thanks a lot :)

OpenStudy (anonymous):

can i have a lot of your Qs more often so i practice on diagnosis

thomaster (thomaster):

I have more of these cases which are assigned to other ppl in my medschool class just a sec ill translate one

thomaster (thomaster):

A skin disease is diagnosed in a patient who has been working in Southeast Asia. The symptoms are raspberry-like sores all over the body and a strong callosity of the footsoles. The tests TP-PA quantitative, VDRL and FTA.abs give a positive result. what is the diagnose?

thomaster (thomaster):

What is the diagnose and in what stage is the disease

OpenStudy (anonymous):

ok i am on it

OpenStudy (anonymous):

the disease is a most likely veneral and as a result could be syphillis or yaws

thomaster (thomaster):

the person on this case diagnosed it a Treponema pallidum infection which can cause syphilis, pinta, bejel and yaws

thomaster (thomaster):

but its most likely syphilis

OpenStudy (anonymous):

yeah should be syphillis

OpenStudy (anonymous):

a next one for the night so i can sleep on it

thomaster (thomaster):

allright

thomaster (thomaster):

damn this one is a pain to translate xD

thomaster (thomaster):

A 22 year old female comes to the GP with symptoms strangury, polyuria and macroscopic haematuria. In the sediment you find a lot of white and red blood cells and some bacteria. after cultivation shows less than 10^4 E.coli. You do a research for chlamydia but the test is negative and you decide to wait. 3 days later the woman calls with high fever and excruciating pain in the kidney area. the GP sends he right to the hospital. What is the diagnose?

OpenStudy (anonymous):

the first thing i ll say is the persons immune system is super weak

thomaster (thomaster):

well i'm gonna sleep

thomaster (thomaster):

if you want to know the diagnose it's behind this link http://bit.ly/4nvBcT

OpenStudy (anonymous):

should be staphilococcus saprophyticus because most of the symptoms fit

OpenStudy (anonymous):

where are you getting these questios from

thomaster (thomaster):

Theyre just some cases from medical microbiology assignments

thomaster (thomaster):

Btw s.saprophyticus is the one indeed

Elsa213 (elsa213):

hehehehe ...... thomaster, trying 2 get answers? BUT CHU CANT MUAHAHAHAHAHA!!!!

thomaster (thomaster):

This was actually my very first question on OpenStudy :P

Elsa213 (elsa213):

i can c dat ;p
