I think maybe 4? I think it's a little redundant to say "A decision was reached" because when you make a decision, it has essentially been "decided/reached" if you know what I mean o_O. I think 4 is better than 5 because you just use one verb "decided" rather than "made a decision" which just seems a bit more wordy...

This is for number one. I think it is c. Because if you put grammatical terms, the word made and to don't actually belong together and so it couldn't be a or d and double negatives would be used in the last one so is in between c or b which both could be used correctly but, I think it is c.
