Yes you need to put the author and the date provided. It helps understand where the contxt is coming from. I would reference it in the front or the back. either one would be fine

Well if your talking phrases from a author thats not yours. The author needs credit. So yes:)

You do not need to put a citation at the end of each sentence if it is the same source that you are quoting, only if you use different sources in between would you cite each sentence. Also, if you are paraphrasing a source, the citation goes where ever the paraphrasing ends and your writing begins. Also if your quote is several sentences (40 or more words), you must use a block quote and cite at the end of the block. 

Also I would not use quoted material for chapter or section titles. Come up with your own titles and then use what you wanted to quote in your body. Also, remember to use page numbers when you quote your source directly, but do not use page numbers when paraphrasing.
