Can anyone help me with "converting between forms" please?

Yes, this question is ambiguous considering that there are many forms for each type of equation in algebra. For example, in respect to the quadratic you can have an equation in general form, f(x)=ax^2+bx+c, or in vertex form f(x)=a(x-h)^2+k. Usually, to convert between forms you must find all information about specific variables depending on the form, usually everything but the vertex or x and y, then depending on the form you are in do a certain set of operations to convert to another form. It really all depends on the type of equation you are speaking of and which forms you want to convert between.
