The problem gives you a coordinate point- "( 1 , -4 )" So you will plug this point into the equation (the point slope form) by labeling x (which is 1) and y (which is 4) as "x1" and "y1".

The slope intercept form is y = mx + b, where m = slope, and b = y-intercept. Look at the 4 choices. Two of them have some expression for y involving 2x. Those two choices can be eliminated since you need to have (1/2)x, not 2x as your slope is 1/2 not 2. Now plug in (1, -4) into the other two choices and see which one works.
