First of all, that is not the first step when you complete the square. Secondly, in the second step, you should get something along the lines of \(\left(x + \frac{1}{2}\right)^2 = \cdots\) as opposed to \((x + 4)^2 = \cdots\).

In the first step, you add \(\left(\frac{b}2\right)^2\) to both sides. As \(b = 1\) in this case, you would add \(\left(\frac{1}{2}\right)^2\) to both sides.
