I think it's the murder of Banquo, Macbeth's "friend". If he wasn't so full of ambition and selfishness then he would have let the prophecy come true, and in the end I think Banquo's son gets away anyway, so killing his father really did nothing. It's definitely the killing of Banquo.

I want to say biblical allusion because it does say something like that in the bible, but I don't think that is the right answer. I think it actually might be personification, because he (in the form of literature) just gave trees the ability to speak and rocks to move. Do rocks ever move or trees actually say hi? no, so the answer is personification.

Hallucinations is definitely one of them. Macbeth sees banquo at the party he held early on in the story shortly after duncan and banquo were both killed. Lady Macbeth starts sleep waking and eventually becomes sick, I believe that it is the lack of sleep that she is getting. That should be the other answer.
