Suppose your health plan covers lab tests in full if you go to an in-network lab, but only pays 40% of allowed charges if you go out of network. You forget to check and go get your blood test at a lab that turns out to be out of network. The lab bills you $110 for the blood test. Your health insurance allows only a $25 charge for that test. How much would you have to pay out of pocket for that lab test?

well, your health insurance for out-of-network services (namely doctors or services outside a grid they work with) , only pays 40% you got billed 110 hmmm but I notice the insurance only allowed $25 meaning, from the 110, they're only paying 25 so you owe the rest 110 - 25 <-- that much
