As related to the other question, ESBL is Extended Spectrum Beta Lactamase. It's a kind of antibiotic resistance in certain strains of bacteria. I've never heard of the term "post ESBL effect" but I would wager that in this context it concerns treatment options in an era after many bacteria, including E. Coli, have developed multiple antibiotic resistance. A little more context may allow me to help further.

You would like to detect ESBL to determine what treatment can or cannot be used. In bacteria, genes conferring resistance may be expressed, or repressed. Silent genes will look susceptible to antibiotic. The use of clavulanic acid (inhibits ESBL) and 3rd generation cephalosporins can help with the detection. The formation of a keyhole around the clavulanic acid in AST confirms ESBL present, whether gene is expressed or repressed.
