High temperatures and/or pH variations. Once denatured it can't come back to its original form.

Causes for protein denature: pH variation, temperature change, mechanical force, heat, and the organic solvent. e.g protein in water cannot be used in lipids.

denaturing is changing the active site of the protein. Factors mentioned by jyro and mellinia are examples of how the active site of the protein can be "denatured" or permenately damaged.
