first of all - skin cancers. different diseases: BCC - basal cell carcinoma. locally invasive but never ever metastatic. as such - local control - simply put - take it all out with clean margins. squamous cell carcinoma - various subtypes. one subtype is a bit more problematic. once again - mainly excision. in both cases a cytotoxic anti metabolite - 5-Fluoro-Uracil may be used topically. melanoma - the trickiest of all. not necessarilly pigmented btw (amelanotic melanoma) this cancer tends to metastasize. quite early in its course. the main predictor is depth of tumor (not size). i think it is called the Breslow classification. once again local control is the basis. sentinel lymph node biopsy may be apropriate in some cases. melanoma is highly resistant to all known chemotherapies. there is an immunomodulatory modality - namely a vaccine made of "meshed tumor" - shows some success. state of the art next generation drugs will aim at a molecular target - BRAF which is a key molecular player in melanoma
