First of all, thank you for an excellent example of how a question like this should be posted. Not only did you provide every bit of relevant information that will be needed to properly answer the question, you also provided a link to the source that you are citing, which is great if there are further questions that one might have in answering your question. Good job! What you have posted up there: Agency for Healthcare Research and Quality. (2007). Closing the Quality Gap: A Critical Analysis of Quality Improvement Strategies: Volume 7—Care Coordination, Structured Abstract. (Publication No. 04(07)-0051-7). Rockville, MD. Is actually pretty close to a proper APA reference page entry. Since there is not author listed you use the name of the organization followed by a period. - Check For the date, change it to (2007, June) followed by a period. For the title, only capitalize the first word of the title and the first word of the subtitle as well as any proper names. So the words that should be capitalized in the title are: "Closing" and "A". Follow the title with a period. Now this does not appear to be a journal article so Volume 7 might be a part of the title. If so, capitalize and include it in the title. Whether the volume part should be included at all is iffy with me, but when in doubt more is better than not enough. Get rid of the structured abstract part. That is just a description of the content of the webpage that you are on. You are citing the article not the abstract. Publication number part is correct except it might need to be enclosed in square brackets instead of parenthesis. I'll have to consult my manual when I get home. Publication location is only needed for books so get rid of it. In the retrieved part, if you are referencing the full article use the URL to that instead of the link to the abstract, otherwise what you have is right. Personally, I would reference the full article rather than the abstract. It's more professional and indicates that you read the whole article and not just the abstract.
