yes you figured it out right j(x) is actually the y coordinate
so from now i`ll call j(x) as y
so general equation is
y = mx + c
where c is the y intercept
now put the values given in the table and find the value of c

yes you generally put it in function notation this way
f(x) = x + 5
but actually when you plot the value which is f(x) is the y coordinate
so it`s totally up to you you can write j(x) or y
