Point slope form is, \[y-y_0=m(x-x_0)\]where, m, the slope can be obtained from, \[m=\frac{y_1-y_0}{x_1-x_0}\]And the points are, as you can see in the graph, \[(x_0,y_0)=(0,1)\]\[(x_1,y_1)=(3,4)\]Now obtain m first, and then substitute them in the point slope form.
