B, you have to have one at the beginning and end of something you're putting in quotation marks!

I would say that both A and B are right, however B would be the "more right" answer because no matter what they are ALWAYS used in pairs where as they CAN be use to emphasize a word, but more often than not you are going to find an emphasized word in italics or capitalized. C Is not likely because when you paraphrase something you leave it without quotes, and D is unlikely because typically titles are underlined not quotationed.
The answer would be C. The only reason I believe this is because when you're paraphrasing you may also ask a question.
