well the shift up is simply caused by adding 5 \[y = \sqrt{x} + 5\]
what bout the 3 units to the left?
well the equation \[y = \sqrt{x} \] has a vertical asymptote at x = 0 so you need to make it move 3 left... so the vertical asymptote is x = -3 so then \[x \ne 3\] rewrite it so its 0 on the right hand side, and that's what you need to take the square root of.
