re ProfBrainstorm: "an ion is an atom that has gained or lost electrons, compared to the neutral atom" - Correct. An Ion is a element with a different charge because of more more or less electrons. The Neutron has nothing to do with the charge so any reference to it would not be the best answer. An element that changed the numbers of proton would not be the same element. A is the correct answer.

yes, you make a fair point @mstoldegon - although when one thinks about the ionisation of an atom there isn't any change to the nucleus, be it the number of protons or neutrons
