Polar->3-D coordinate system conversion

They always like to throw in extra bits of info in my lesson, like this problem. Please notice there is not detailed solution for this so the only one I have a a graps on is the r=sqrt(x^2+y^2+z^2) but even that one idk where it comes from (because the x and y have extra terms on it, not like polar)
