Boiling water is a physical change. all changes of state are physical changes

What physical changes usually do is to retain identity for substance. Ex: Change of melting points is considered a physical change because there's a change in temperature (warm or cold) and change of state (boiling point changes ice to water)

That was for question 2. For one, you already know the physical change so you want to know how chemical changes occur. Chemical changes occurs when substances are converted into a new substance. Example, paper gets burn, before the combustion, it was a paper, afterwards it no longer becomes paper (ashes).
