standard form is y=mx+b mx is your slope b is your y intercept. the line intercepts y at 4 so now it becomes y=mx+4 then how far away is (0,4) from (2,8)? you go up 4 then over 2 that gives you 4 over 2 simplify that and it gives you 2 your slope will always have x next to it so it becomes 2x so your new equation is y=2x+4. make sense?
