Cassius, who is speaking the lines, is one of the conspirators against Caesar. So you can rule out "great man" and "hard worker". Similarly, "liar" can be ruled out because Caesar is not presented as saying anything which is untrue. The word "coward" makes you think that "fearful person" is what Cassius thought. However, his words are merely the typical prejudices people had (have?) against certain illnesses. Caesar had health problems, possibly migraine headaches, epilepsy and/or the aftermath of malaria.
