This is from the web. Adaptations of a frog: A frog has two long hind legs that help it jump. ... A frog has two short front legs that help prop the frog up when it sits or gets ready to jump. ... A frog's feet are webbed, so it is a good swimmer. A frog's eyes and nose are on top of its head. ... A frog has two bulging eyes. Each segment on an earthworm's body has a number of bristly hairs, called setae (sometimes written as chaetae). ... An earthworm has a streamlined body with no antennae or fins or arms or legs! ... An earthworm has circular muscles that surround each body segment. Squid have very good eyesight and may even be able to see in color. The squids' two tentacles are specially adapted for feeding and they use them to grab their prey. They have a sharp beak on their mouths that they use to break open shells. Squid have some unique adaptations.

...keeping all this in mind, one creature like this would be an octopus: eyes and nose on top of head (from frog) has two bulging eyes (from frog) some types of octopus have bristly hairs (from worm) circular muscles that surround each body segment (from worms) - however different, the tentacles begin as “circular muscles” that surround the body, quite similar don’t you think? very good eyesight (squid) tentacles (squid) sharp beak and mouth (squid) I personally think that the octopus has the best features of each, but up to you, keep in mind there are other correct answers for this question... happy studying! :)
