Help me find out how to write this equation in point slope form?

OpenStudy (spanishrose):

The question says I have a slope of 3 and a y intercept with the coordinates (0,12). It told me to write it in slope intercept form, so I obviously used the y=mx+b formula and got y=3x+12

OpenStudy (spanishrose):

But then it told me to write in a different way, in point slope form, which idk how to do... can anyone help?

OpenStudy (abbles):

Alrighty! Point-slope form is y-y1 = m(x - x1) where m is the slope, y1 is the y-coordinate of your point and x1 is the x-coordinate of your point. Now plug in the values you know!
