Is POH+PH=14 always? Or is there any condition to use it? Please explain.

its relative scale :) absolute scale is Ka values :) this PH and POH is just for the free convenience :) to say the acid or base

maybe ,,,,, they found that concentration of H+ ions in water was 10^-7 M and since water is neutral ,, the concentration of OH- also must be the same.........so taking -ve log and adding up ==> 14 (just guessing)
