Which of the following statements is true about ions? a. They are formed as electrons move around inside the atom. b. Cations and anions are always the exact same size. c. Cations are negatively charged and anions are positively charged. d. Cations are positively charged and anions are negatively charged.

d is the answer. Basically, cation are ions formed from metals, in which they lose electrons from their valence shell, therefore being in excess of protons and so being considered to be overall positively charged and since their valence shell vanishes, their size dimishes, whereas for anions, these are made when atoms gain electrons, increasing the size of the atom itself.
